Community Care Services Program (CCSP), helps senior adults and people with functional and/or cognitive impairments stay in their homes and communities for as long as possible. CCSP is a program of the Georgia Department of Community Health.
The program is designed for frail, elderly, and disabled Georgians who require the level of care typically provided in a nursing home. However, this program allows eligible individuals to receive care in their homes or communities (such as in assisted living facilities, also referred to as personal care homes) and avoid having to use long-term nursing home care. For some very low-income participants, this program provides both medical care and non-medical personal care services.
The GAPP program is operated through Medicaid. It was developed to accommodate medically fragile children under the age of 21. The Georgia Pediatric Program delivers skilled nursing care in Georgia to medically fragile children. Personalized care provided in the privacy of the child’s home is administered by skilled nursing professionals and home health aides. Recipients of the GAPP program get assistance with daily living activities and specialized medical support.
The GAPP program requires that children be medically fragile and diagnosed accordingly. All participants of the program must be under 21. Care administered by qualified professionals address the fragile needs of children who are eligible for admission to a hospital or long-term skilled nursing care facility. The Alliant Georgia Medical Care Foundation’s (GMCF) Medical Review Team conducts the review and makes the determination.
